The MyCVMC Patient Portal gives patients a secure and convenient, 24-hour access to personal health information and medical records for patients who have had a visit to Central Valley Medical Center or one of our clinics since May of 2014.


How do I create an account with Self-Enrollment?

  • To create your account please click on “CREATE YOUR ACCOUNT” above and complete the self-enrollment form. Patients will need their full social security number to complete the self-enrollment form.
  • Next it will ask you to verify the patient match found is you and agree to terms of use.
  • Then enter your email under “I Need an Account” and click Sign Up.
  • Once your account is created, you will need to complete the Creat an Account information form and create a Username, Password, and security question.

Is my information secure in MyCVMC?

Yes; MyCVMC takes great care to ensure your health information is kept private and secure. Central Valley Medical Center uses Cerner Health to provide a secure username and password used to access your patient record information on MyCVMC.

MyCVMC Access for Caregiver Relationship.

Parents of children may link children health records, or with their consent other adult family members to their personal patient portal account. Please contact your family physician’s office to receive an invitation to setup access.

Central Valley Medical Center is dedicated to providing resources to help you better manage your health. We are excited to announce that our MyCVMC Patient Portal works with the Apple Health app, which is available to patients with an iPhoneTM running iOS 11.3 or later; and available to use on any Andriod phone with access to Coral Health Records App.

Apple Health App Setup

Setup is simple. Accessing your health records from Central Valley Medical Center and its clinics through your iPhoneTM is as easy as

  • going to the Health Data section and selecting Health Records on the Health app on your iPhone,
  • add an account and search for a CVMC institution,
  • and then log in with the user name and password that you normally use for MyCVMC Patient Portal

After you log in once, your health records will start to appear in the Health app, and you will be notified when new records are ready to view.
